Structural Heart Devices Market: How Coronary Access Innovation Is Reshaping TAVR

Heart valves don't often make headlines, but the numbers behind the devices that repair them are hard to ignore. The global structural heart devices market was valued at USD 18.6 billion in 2025 and is on track to reach USD 21.1 billion in 2026, before climbing to USD 51.0 billion by 2033 — a compound annual growth rate of 13.5% between 2026 and 2033. That's a market expected to nearly 2.5x in under a decade, and the reasons why sit at the intersection of an aging population, shifting surgical philosophy, and a wave of device innovation that's redefining what counts as "high-risk" for heart surgery.

Why the Market Is Accelerating

The core growth story isn't really about devices — it's about disease burden and a change in how doctors treat it. Cardiovascular disease remains staggeringly common. A January 2025 Oxford Academic analysis put the global age-standardized prevalence of cardiovascular disease at roughly 7,179 cases per 100,000 people, underscoring how widespread heart-related conditions still are worldwide. In the U.S. specifically, the American Heart Association reported 931,578 cardiovascular-related deaths as of January 2024 — nearly 3,000 more than the year before — with the age-adjusted death rate rising 4.0% to 233.3 per 100,000 people.

Valvular heart disease is a major slice of that burden. Research from the Cardiovascular Research Foundation's PREVUE-VALVE study found that 4.7 million Americans aged 65–85 are living with moderate to severe valvular heart disease, while rheumatic heart disease affects an estimated 54.8 million people globally. Numbers like these explain why transcatheter valves and left atrial appendage closure (LAAC) devices have moved from niche interventions to mainstream cardiology tools — they offer a way to treat high-risk, often elderly patients without the trauma of open-heart surgery.

The bigger structural shift, though, is regulatory. TAVR was originally approved only for patients too frail for surgery. Over the past several years, clinical guidelines have expanded it to intermediate- and even low-risk patients, turning what was once a last resort into something closer to a first-line option. That single change has arguably done more to expand the addressable market than any single product launch.

Product Segments: TAVR's Dominance and What's Chasing It

Among device categories, Transcatheter Aortic Valve Replacement (TAVR) leads by a wide margin, accounting for 53.4% of global revenue in 2025. That dominance reflects both procedural volume and continued engineering refinement — Medtronic's Evolut FX+, for instance, received CE Mark approval in October 2024 with coronary access windows roughly four times larger than earlier Evolut models, a change aimed squarely at making future coronary interventions easier for patients who receive TAVR earlier in life.

Surgical Aortic Valve Replacement (SAVR) isn't disappearing, though — it's evolving alongside TAVR rather than being replaced by it. Corcym's FDA-approved Perceval Plus, cleared in March 2025, brought laser-cut leaflets and a zero-pressure fixation process designed to reduce mechanical stress and extend valve longevity, a reminder that surgical valves are still attracting real R&D investment even as transcatheter approaches grow faster. Beyond the aortic valve, mitral repair (annuloplasty) and left atrial appendage closure round out the market's structure, each addressing a different anatomical failure point but sharing the same underlying push toward catheter-based, minimally invasive delivery.

Regional Picture: North America Leads, Asia Pacific Accelerates

North America held the largest share of the market in 2025, at 52.5% of global revenue, with the U.S. as the clear leader within the region. Part of that dominance is structural — the U.S. has high CVD prevalence and dense specialist infrastructure — but part of it is also deliberate policy support. In October 2024, the CDC awarded USD 8.4 million in grants to the American Heart Association specifically to run national heart-valve-disease education campaigns, a signal that public health agencies see awareness, not just device access, as a growth lever.

Asia Pacific, meanwhile, is set to grow at the fastest rate through 2033 as healthcare infrastructure matures and transcatheter procedures become more widely reimbursed and available outside tertiary hospitals in major economies like Japan, China, and India.

Who's Building These Devices

The competitive landscape is concentrated among a handful of established medtech names rather than spread across dozens of players, which itself tells you something about the capital intensity of this space — structural heart devices require years of clinical trial data before regulators will approve them. Edwards Lifesciences and Medtronic effectively anchor the TAVR category, trading milestones back and forth: Edwards secured FDA approval in May 2025 for its SAPIEN 3 platform to treat asymptomatic severe aortic stenosis, the first such approval for asymptomatic patients, while Medtronic followed with CE Mark clearance the same month for valve-in-valve procedures using its Evolut PRO+ and Evolut FX systems, regardless of which manufacturer made the original failed valve. Boston Scientific and Abbott Laboratories compete strongly in the mitral repair and LAAC segments, while St. Jude Medical, LivaNova, JenaValve Technology, CardioKinetix, Biomerics, and Comed BV round out a field defined more by specialization than sheer scale.

The Friction Points

None of this growth is frictionless. The biggest restraint is cost. TAVR and comparable structural devices demand precision engineering and biocompatible materials that are expensive to source and manufacture, and that cost is compounded by the complexity of the procedures themselves — specialized hybrid operating rooms, trained interventional cardiology teams, and imaging support don't come cheap. In lower-income markets, this pricing reality slows adoption even where disease prevalence is high, which is part of why growth remains uneven across regions despite a universally rising CVD burden.

The Bigger Picture

What makes this market worth watching isn't just the 13.5% CAGR — it's that the growth is coming from a genuine change in medical practice, not just volume. As TAVR moves into younger, lower-risk populations, the entire value proposition of structural heart devices shifts from "last resort for the frail" to "standard pathway for aortic stenosis," and that reclassification alone could keep reshaping demand well past 2033.
